Comprehensive Health Risk Assessment (CHRA)

Sapphire Care Management has conducted more than 300,000 extensive health risk assessments for Medicare Advantage plans in Puerto Rico. Our team of highly qualified physicians is capable of engaging health plan members across both urban and rural areas in Puerto Rico. We conduct annual health profiles and comprehensive health risk assessments either in the member’s home or through telemedicine services.

Transition of Care

Sapphire Care Management orchestrates seamless transition of care services for health plan members recently discharged from medical facilities with a heightened risk of readmission. In the aftermath of a hospital discharge for a member deemed high-risk for readmission, a Sapphire Care Management physician undertakes a home visit, prioritizing a positive care outcome to mitigate post-hospital complications and curtail hospital readmissions. During the home visit, the physician engages in the following activities:

  • Conducts a comprehensive post-discharge health assessment.
  • Executes a meticulous Medication Review Post Discharge (MRPD) with the primary objective of averting complications and hospital readmissions attributed to medication-related issues such as duplication, adverse drug events, errors, and non-adherence.
  • Confirms the presence of ordered equipment or health services at the member’s home, as prescribed by the admitting hospital physician
  • Ensures that the member has coordinated a post-discharge medical appointment with their attending physician.

Discharge Planning

Sapphire Care Management extends its Discharge Planning Program to hospitals and medical plans alike. This comprehensive service includes bedside intervention throughout the member’s hospital stay, ensuring a seamless coordination with our outpatient transition of care program.
